To pay for your Prevention Rx workshop registration online, please follow these instructions:

Step 1: Fill out the Prevention Rx registration form and submit to NC Prevention Partners via fax or mail. If paying by check, be sure to include your payment. If you prefer to pay online using Paypal, continue to Step 2.

Step 2: Please choose your online payment option below. (If you are registering more than one person from the same office or clinic, be sure to add the correct number of registrations to your cart.)

Step 3: Complete your online payment by clicking on the 'View Cart' button below and proceeding to the secure checkout.

Note: Your registration will not be complete until we receive both your registration form and your payment (either a check or Paypal online payment).

Individual Registration - $85.00

Two or more people from the same office/clinic (must register at the same time) - $75.00/person

Late Registration (within one week of the workshop) - $125.00
