Tools for your School Wellness Team

The Zone Health Program provides several tools to help your school wellness team implement healthy changes in food and physical activity on campus:

The Assessment Grid: 

Elementary Middle School High School

This grade-specific document can be used to evaluate policies and practices affecting nutrition, nutrition education, physical activity and physical education in your school. It is designed to be completed by one or more members of your team who are knowledgeable about day-to-day operation of programs and classes that impact good nutrition and physical activity opportunities. The assessment grid evaluates 27 areas of wellness within the school and assigns a letter grade for each area assessed. The assessment tool will help schools identify areas to target for improvement.

The Action Plan:

The Action Plan is a worksheet designed to organize your wellness team's ideas about how to make changes in your school's wellness environment, targeting the priority areas identified in the your school's assessment. It guides your team in writing short and long term goals or tasks that put your plan into action.
